[:es]Trastornos Hipertensivos Agudos en Pacientes Embarazadas[:en]Acute Hypertensive Definitions in the Pregnant Patient[:]

[:es]Los trastornos hipertensivos son comunes durante el embarazo y se pueden clasificar en cuatro categorías asociadas con el embarazo: (1) hipertensión crónica, (2) hipertensión gestacional, (3) preeclampsia, y (4) hipertensión crónica con preeclampsia superpuesta. Además, pueden ocurrir emergencias hipertensivas no relacionadas con el embarazo en la paciente embarazada.

Trastornos Hipertensivos Agudos en Pacientes Embarazadas

Nombre Criterios PA Criterios Adicionales
Hipertensión aguda “Severa” PAS ≥ 160 mm Hg o PAD ≥ 110 mm Hg
Preeclampsia PAS ≥ 140 mm Hg o PAD ≥ 90 mm Hg Las lecturas de PA deben ocurrir en ≥ 2 ocasiones, con ≥ 4 horas de diferencia, > 20 semanas de gestación
Ya sea:
• Proteinuria (recolección de orina de 24 horas ≥300 mg de proteí­na)
O la recolección de orina puntual Uproteí­na/UCr ≥0.3 mg/dL)
• Características severasa
Eclampsia Grado de preeclampsia de elevación de la PA Convulsiones de gran mal de reciente comienzo en una mujer sin trastorno convulsivo conocido
Síndrome HELLP Con o sin grado de  preeclampsia de elevación de la PA Evidencia de lo siguiente:
• Hemólisis (esquistocitos en frotis periférico, aumento de LDH, disminución de haptoglobina, aumento de bilirrubina total [≥ 1,2 mg/dL], disminución de hematocrito)
• Elevación de las enzimas hepáticas GOT/GPT (≥ 70 UI/L)
• Plaquetas bajas (<100.000 mcL)
Emergencia hipertensiva PA ≥ 240/140 mm Hg

 

a Características severas = PAS ≥ 160 mm Hg o PAD ≥ 110 mm Hg, Plt <100,000/mm3, GOT/GPT >2 x LSN, dolor del cuadrante superior derecho o epigástrico que no responde a la medicación, síntomas cerebrales/visuales, lesión renal (Cr > 1,1 mg/dL o >2 veces el valor basal) o edema pulmonar.

Abreviaturas: UCr = creatinina urinaria; LSN = límite superior de la normalidad; Uproteína = proteína urinaria; PAS = presión arterial sistólica; PAD = presión arterial diastólica.

Creado Nov 21, 2019.[:en]Hypertensive disorders are common during pregnancy and can be classified into four pregnancy-associated categories: (1) chronic hypertension, (2) gestational hypertension, (3) preeclampsia, and (4) chronic hypertension with superimposed preeclampsia. In addition, non-pregnancy associated hypertensive emergencies can occur in the pregnant patient.

Acute Hypertensive Definitions in the Pregnant Patient

aSevere features = SBP ≥ 160 mm Hg or DBP ≥ 110 mm Hg, Plt < 100,000/mm3, AST/ALT > 2 x ULN, right upper quadrant or epigastric pain unresponsive to medication, cerebral/visual symptoms, renal injury (SCr > 1.1 mg/dL or > 2 x baseline), or pulmonary edema.

Abbreviations: Tbili = total bilirubin; UCr = urinary creatinine; ULN = upper limit of normal; Uprotein = urinary protein; SBP = systolic blood pressure; DBP = diastolic blood pressure.
