[:es]Las intervenciones coronarias percutáneas (PCI) incluyen la angioplastia coronaria transluminal percutánea (ACTP) con o sin colocación de stents. Las indicaciones fundamentales son el tratamiento de la angina de pecho (estable o inestable), isquemia miocárdica, infarto agudo de miocardio (sobre todo en pacientes con shock cardiogénico en desarrollo o establecidos).

Indicaciones y contraindicaciones clínicas para PCI
Indicaciones

  • Infarto agudo de miocardio con elevación del ST (STEMI)
  • Síndrome coronario agudo sin elevación del segmento ST
  • Angina estable
  • Equivalente anginoso (por ejemplo, disneaarritmiamareos/síncope)
  • Pacientes asintomáticos o ligeramente sintomáticos con pruebas objetivas de una moderada a amplia zona de miocardio viable o isquemia moderada a severa en pruebas no invasivas

Contraindicaciones

  • Comorbilidades significativas (contraindicación relativa)

 

Indicaciones y contraindicaciones angiográficas a PCI
Indicaciones

  • Lesión significativa hemodinámicamente en un vaso que irriga miocardio viable (diámetro del vaso > 1,5 mm)

Contraindicaciones relativas

  • Estenosis en rama principal izquierda en un paciente que es candidato para cirugía
  • Arteria de pequeño calibre difusamente enferma o vena de injerto
  • Otra anatomía coronaria no susceptible a intervención percutánea

Creado: Feb 18, 2011.[:en]Percutaneous coronary interventions (PCI) include percutaneous transluminal coronary angioplasty (PTCA) with or without stenting. Primary indications are treatment of angina pectoris (stable or unstable), myocardial ischemia, and acute MI (particularly in patients with developing or established cardiogenic shock).

Clinical Indications and Contraindications to PCI
Indications

  • Acute ST elevation myocardial infarction (STEMI)
  • Non ST elevation acute coronary syndrome
  • Stable angina
  • Anginal equivalent (eg, dyspnea, arrhythmia, dizziness/syncope)
  • Asymptomatic or mildly symptomatic patients with objective evidence of a moderate to large area of viable myocardium or moderate to severe ischemia on noninvasive testing

Contraindications

  • Significant comorbidities (relative contraindication)

Angiographic Indications and Contraindications to PCI
Indications

  • Hemodynamically significant lesion in a vessel serving viable myocardium (vessel diameter >1.5 mm)

Relative contraindications

  • Left main stenosis in a patient who is a surgical candidate
  • Diffusely diseased small-caliber artery or vein graft
  • Other coronary anatomy not amenable to percutaneous intervention
