{"id":2123,"date":"2019-08-16T20:57:47","date_gmt":"2019-08-16T20:57:47","guid":{"rendered":"https:\/\/medicalcriteria.com\/web\/?p=1762"},"modified":"2026-01-30T19:19:44","modified_gmt":"2026-01-30T19:19:44","slug":"raynaud","status":"publish","type":"post","link":"https:\/\/medicalcriteria.com\/web\/raynaud\/","title":{"rendered":"Diagnostic Criteria for Primary Raynaud\u2019s Phenomenon (RP)"},"content":{"rendered":"<div class=\"99c380e4b4a7b96c35d7ddf7dcb434e8\" data-index=\"1\" style=\"float: none; margin:0px 0 0px 0; text-align:center;\">\n<script async src=\"https:\/\/pagead2.googlesyndication.com\/pagead\/js\/adsbygoogle.js\"><\/script>\r\n<!-- MC 2019- Horizontal -->\r\n<ins class=\"adsbygoogle\"\r\n     style=\"display:block\"\r\n     data-ad-client=\"ca-pub-0127150553352455\"\r\n     data-ad-slot=\"3806776041\"\r\n     data-ad-format=\"auto\"\r\n     data-full-width-responsive=\"true\"><\/ins>\r\n<script>\r\n     (adsbygoogle = window.adsbygoogle || []).push({});\r\n<\/script>\n<\/div>\n<p>Raynaud\u2019s phenomenon (RP), is a disorder of the microvasculature that generally affects the fingers and toes but can present on other extremities such as the nose, ears and nipples.<!--more--><\/p>\n<p><strong>Diagnostic Criteria for Primary Raynaud\u2019s Phenomenon<\/strong><\/p>\n<ul>\n<li>Meets 3 Step Criteria for diagnosis of Raynaud\u2019s phenomenon<\/li>\n<li>Normal Capillaroscopy (e.g. clusters 1 and 2, which were described as \u201cnormal\u201d and \u201cperfect normal\u201d)<\/li>\n<li>Physical examination is negative for findings suggestive of secondary causes, (e.g.ulcerations, tissue necrosis or gangrene, sclerodactyly, calcinosis, or skin fibrosis)<\/li>\n<li>No history of existing connective tissue disease<\/li>\n<li>Negative OR low titer ANA (e.g. 1:40 by indirect immunofluorescence)<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>Three-step approach to diagnosis of Raynaud\u2019s Phenomenon<\/strong><\/p>\n<p><strong>Step 1:<\/strong> Ask screening Question<br \/>\nAre your fingers unusually sensitive to cold?<br \/>\nYes, proceed to step 2<\/p>\n<p><strong>Step 2:<\/strong> Assess color changes<br \/>\nOccurrence of biphasic color changes during the vasospastic episodes (white and blue)<br \/>\nYes, proceed to step 3<\/p>\n<p><strong>Step 3:<\/strong> Calculate disease score<\/p>\n<ol style=\"list-style-type: lower-alpha;\">\n<li>Episodes are triggered by things other than cold (i.e. emotional stressors)<\/li>\n<li>Episodes involve both hands, even if the involvement is asynchronous and\/or asymmetric<\/li>\n<li>Episodes are accompanied by numbness and\/or paresthesias<\/li>\n<li>Observed color changes are often characterized by a well-demarcated border between affected and unaffected skin.<\/li>\n<li>Patient provided photograph(s) strongly support a diagnosis of RP.<\/li>\n<li>Episodes sometimes occur at other body sites (e.g. nose, ears, feet, and areolas).<\/li>\n<li>Occurrence of triphasic color changes during the vasospastic episodes (white, blue, red)<\/li>\n<\/ol>\n<p>If 3 or more criteria met from Step 3 (a &#8211; g), then the patient has RP<\/p>\n<p>&nbsp;<\/p>\n<p><strong>References:<\/strong><\/p>\n<ol>\n<li>Stringer T, Femia AN.
