Usual interstitial pneumonia (UIP) is the histopathological pattern of IPF. IPF is characterized by progressive worsening of dyspnea and lung function and is associated with a poor prognosis.<!--more--><\/p>\n<p><strong>Diagnostic Criteria for Idiopathic Pulmonary Fibrosis (IPF)<\/strong><\/p>\n<p>Diagnosis of IPF requires the following:<\/p>\n<ol>\n<li>Exclusion of other known causes of interstitial lung disease (ILD) (e.g., domestic and occupational environmental exposures, connective tissue disease (CTD), drug toxicity), and either #2 or #3:<\/li>\n<li>The presence of the HRCT pattern of UIP (Table 1)<\/li>\n<li>Specific combinations of HRCT patterns (Table 1) and histopathology patterns (Table 2) in patients subjected to lung tissue sampling<\/li>\n<\/ol>\n<p><strong>Table 1. <\/strong><strong>High-Resolution Computed Tomography (HRCT) Scanning Patterns<\/strong><\/p>\n<table style=\"border-collapse: collapse; width: 100%;\" border=\"1\">\n<tbody>\n<tr>\n<td style=\"width: 92px;\">UIP<\/td>\n<td style=\"width: 472px;\">\n<ul>\n<li>Subpleural and basal predominant; distribution is often heterogeneous (Variants of distribution: occasionally diffuse, may be asymmetrical)<\/li>\n<li>Honeycombing with or without peripheral traction bronchiectasis or bronchiolectasis (Superimposed CT features: mild ground-glass opacities, reticular pattern, pulmonary ossification)<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 92px;\">Probable UIP<\/td>\n<td style=\"width: 472px;\">\n<ul>\n<li>Subpleural and basal predominant; distribution is often heterogeneous<\/li>\n<li>Reticular pattern with peripheral traction bronchiectasis or bronchiolectasis<\/li>\n<li>May have mild ground-glass opacities (GGO)<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 92px;\">Indeterminate for UIP<\/td>\n<td style=\"width: 472px;\">\n<ul>\n<li>Subpleural and basal predominant<\/li>\n<li>Subtle reticulation; may have mild ground-glass opacities or distortion (\u201cearly UIP pattern\u201d)<\/li>\n<li>CT features and\/or distribution of lung fibrosis that do not suggest any specific etiology (\u201ctruly indeterminate for UIP\u201d)<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 92px;\">Alternative Diagnosis<\/td>\n<td style=\"width: 472px;\">Findings suggestive of another diagnosis, including:<\/p>\n<ul>\n<li>CT features:\n<ul style=\"list-style-type: circle;\">\n<li>Cysts<\/li>\n<li>Marked mosaic attenuation<\/li>\n<li>Predominant GGO<\/li>\n<li>Profuse micronodules<\/li>\n<li>Centrilobular nodules<\/li>\n<li>Nodules<\/li>\n<li>Consolidation<\/li>\n<\/ul>\n<\/li>\n<li>Predominant distribution:\n<ul style=\"list-style-type: circle;\">\n<li>Peribronchovascular<\/li>\n<li>Perilymphatic<\/li>\n<li>Upper or mid-lung<\/li>\n<\/ul>\n<\/li>\n<li>Other:\n<ul style=\"list-style-type: circle;\">\n<li>Pleural plaques (consider asbestosis)<\/li>\n<li>Dilated esophagus (consider CTD)<\/li>\n<li>Distal clavicular erosions (consider rheumatoid arthritis)<\/li>\n<li>Extensive lymph node enlargement (consider other etiologies)<\/li>\n<li>Pleural effusions, pleural thickening (consider CTD\/drugs)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>&nbsp;<\/p>\n<p><strong>Table 2. <\/strong><strong>Histopathology Patterns and Features<\/strong><\/p>\n<table style=\"border-collapse: collapse; width: 100%;\" border=\"1\">\n<tbody>\n<tr>\n<td style=\"width: 94px;\">UIP<\/td>\n<td style=\"width: 470px;\">\n<ul>\n<li>Dense fibrosis with architectural distortion (i.e., destructive scarring and\/or honeycombing)<\/li>\n<li>Predominant subpleural and\/or loose fibrosis paraseptal distribution of fibrosis<\/li>\n<li>Patchy involvement of lung parenchyma by fibrosis<\/li>\n<li>Fibroblast foci<\/li>\n<li>Absence of features to suggest an alternate diagnosis<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 94px;\">Probable UIP<\/td>\n<td style=\"width: 470px;\">\n<ul>\n<li>Some histologic features from UIP are present but to<br \/>\nan extent that precludes a definite diagnosis of UIP\/IPF<\/li>\n<\/ul>\n<p style=\"padding-left: 40px;\">And<\/p>\n<ul>\n<li>Absence of features to suggest an alternative<\/li>\n<\/ul>\n<p style=\"padding-left: 40px;\">Or<\/p>\n<ul>\n<li>Honeycombing only<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 94px;\">Indeterminate for UIP<\/td>\n<td style=\"width: 470px;\">\n<ul>\n<li>Fibrosis with or without architectural distortion, with features favoring either a pattern other than UIP or features favoring UIP secondary to another cause*<\/li>\n<li>Some histologic features from UIP, but with other features suggesting an alternative diagnosis\u2020<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td style=\"width: 94px;\">Alternative Diagnosis<\/td>\n<td style=\"width: 470px;\">\n<ul>\n<li>Features of other histologic patterns of idiopathic interstitial pneumonia (e.g., absence of fibroblast foci or loose fibrosis) in all biopsies<\/li>\n<li>Histologic findings indicative of other diseases (e.g., hypersensitivity pneumonitis, Langerhans cell histiocytosis, sarcoidosis, lymphangioleiomyomatosis)<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>*Granulomas, hyaline membranes (other than when associated with acute exacerbation of IPF, which may be the presenting manifestation in some patients), prominent airway-centered changes, areas of interstitial inflammation lacking associated fibrosis, marked chronic fibrous pleuritis, organizing pneumonia.
