Most nodules are benign; therefore, many nodules are biopsied to identify the small number that are malignant or require surgery for a definitive diagnosis. In 2012, the ACR convened committees to (1) provide recommendations for reporting incidental thyroid nodules, (2) develop a set of standard terms (lexicon) for ultrasound reporting, and (3) propose a TI-RADS on the basis of the lexicon.<br \/>\n<!--more--><\/p>\n<p><b>Thyroid Imaging Reporting and Data System (TI-RADS)<\/b><br \/>\nThe ultrasound features in the ACR TI-RADS are categorized as benign, minimally suspicious, moderately suspicious, or highly suspicious for malignancy. Points are given for all the ultrasound features in a nodule, with more suspicious features being awarded additional points.<\/p>\n<p>Composition<\/p>\n<ul>\n<li>Cystic or almost\u00a0completely cystic:<\/li>\n<li>Mixed cystic<\/li>\n<li>Solid or almost<\/li>\n<\/ul>\n<p>Echogenicity<\/p>\n<ul>\n<li>Anechoic: 0 points<\/li>\n<li>Hyperechoic or\u00a0isoechoic:<\/li>\n<li>Hypoechoic: 2 points<\/li>\n<li>Very hypoechoic: 3 points<\/li>\n<\/ul>\n<p>Shape<\/p>\n<ul>\n<li>Wider-than-tall: 0 points<\/li>\n<li>Taller-than-wide: 3 points<\/li>\n<\/ul>\n<p>Margin<\/p>\n<ul>\n<li>Smooth: 0 points<\/li>\n<li>Ill-defined: 0 points<\/li>\n<li>Lobulated or\u00a0irregular:\u00a02 points<\/li>\n<li>Extra-thyroidal\u00a0extension:\u00a03 points<\/li>\n<\/ul>\n<p>Echogenic foci<\/p>\n<ul>\n<li>None or large comet-tail artefacts: 0 points<\/li>\n<li>Macrocalcifications: 1 point<\/li>\n<li>Peripheral (rim)\u00a0calcifications:\u00a02 points<\/li>\n<li>Punctate echogenic\u00a0foci:\u00a03\u00a0points<\/li>\n<\/ul>\n<p>Based on their total points, nodules are then classed into five categories of suspicion for cancer:<\/p>\n<ul>\n<li>0 points, benign: No fine-needle aspiration (FNA).<\/li>\n<li>2 points, not suspicious: No FNA.<\/li>\n<li>3 points, mildly suspicious: FNA if &gt;\/= 2.5 cm; follow if\u00a0<span style=\"font-size: 12.16px;\">&gt;\/=<\/span>\u00a01.5 cm.<\/li>\n<li>4 to 6 points, moderately suspicious: FNA if\u00a0<span style=\"font-size: 12.16px;\">&gt;\/=<\/span>\u00a01.5 cm; follow if\u00a0<span style=\"font-size: 12.16px;\">&gt;\/=<\/span>\u00a01 cm.<\/li>\n<li><span style=\"font-size: 12.16px;\">&gt;\/=<\/span>\u00a07 points, highly suspicious: FNA if\u00a0<span style=\"font-size: 12.16px;\">&gt;\/=<\/span>\u00a01 cm; follow if\u00a0<span style=\"font-size: 12.16px;\">&gt;\/=<\/span>\u00a00.5 cm.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><b>References:<\/b><\/p>\n<ol>\n<li>Vargas-Uricoechea H, Meza-Cabrera I, Herrera-Chaparro J.
