Thus, a diagnosis based only on clinical symptoms is unreliable. Early\u00a0laboratory confirmation of clinical diagnosis may be valuable because some patients\u00a0progress over a short period from mild to severe disease and sometimes to death. Early\u00a0intervention may be life-saving.<br \/>\n<!--more--><br \/>\nBefore day 5 of illness, during the febrile period, dengue infections may be diagnosed\u00a0by virus isolation in cell culture, by detection of viral RNA by nucleic acid amplification\u00a0tests (NAAT), or by detection of viral antigens by ELISA or rapid tests. Virus isolation in\u00a0cell culture is usually performed only in laboratories with the necessary infrastructure\u00a0and technical expertise.\u00a0NS1 antigen detection kits now becoming commercially available can be used in\u00a0laboratories with limited equipment and yield results within a few hours.<br \/>\nAfter day 5, dengue viruses and antigens disappear from the blood coincident with the\u00a0appearance of specific antibodies. NS1 antigen may be detected in some patients for\u00a0a few days after defervescence. Dengue serologic tests are more available in dengue-endemic\u00a0countries than are virological tests.<br \/>\nFor serology, the time of specimen collection is more flexible than that for virus isolation\u00a0or RNA detection because an antibody response can be measured by comparing a\u00a0sample collected during the acute stage of illness with samples collected weeks or months\u00a0later. Low levels of a detectable dengue IgM response \u2013 or the absence of it \u2013 in some\u00a0secondary infections reduces the diagnostic accuracy of IgM ELISA tests.<br \/>\nA four-fold or greater increase in antibody levels measured by IgG ELISA or by\u00a0haemagglutination inhibition (HI) test in paired sera indicates an acute or recent flavivirus\u00a0infection.<br \/>\n<b><\/b><\/p>\n<p><b>Interpretation of Dengue Diagnostic Tests<\/b><br \/>\n<b><\/b><\/p>\n<p><b>Highly suggestive\u00a0<\/b><br \/>\nOne of the following:<\/p>\n<ol>\n<li>IgM + in a single serum sample<\/li>\n<li>IgG + in a single serum sample with a\u00a0\u00a0HI titre of 1280 or greater<\/li>\n<\/ol>\n<p><b>Confirmed<\/b><br \/>\nOne of the following:<\/p>\n<ol>\n<li>PCR +<\/li>\n<li>Virus culture +<\/li>\n<li>IgM seroconversion in paired sera<\/li>\n<li>IgG seroconversion in paired sera or fourfold\u00a0IgG titer increase in paired sera<\/li>\n<\/ol>\n<p>Both the identification of virus\/viral RNA\/viral antigen and the detection of an antibody\u00a0response are preferable for dengue diagnosis to either approach alone<\/p>\n<p>&nbsp;<\/p>\n<p><b>References:<\/b><\/p>\n<ol>\n<li>Laughlin CA, Morens DM, Cassetti MC, Costero-Saint Denis A, San Martin JL, Whitehead SS, Fauci AS.
