The consequences of this include lymphoproliferative disease, manifested by lymphadenopathy, hepatomegaly, splenomegaly, and an increased risk of lymphoma, as well as autoimmune disease, typically involving blood cells.<\/p>\n<p><!--more--><\/p>\n<p><strong>Diagnostic criteria for ALPS<\/strong><\/p>\n<p><strong>Required<\/strong><\/p>\n<ol>\n<li>Chronic nonmalignant lymphoproliferation<\/li>\n<li>Defective in vitro Fas-mediated lymphocyte apoptosis<\/li>\n<li>At least 1% TCR {alpha}\/{beta}+, CD3+, CD4-, CD8- cells (DNTs) in peripheral blood or lymphoid tissue<\/li>\n<\/ol>\n<p><strong> Supporting <\/strong><\/p>\n<ol>\n<li>Autoimmune antibodies<\/li>\n<li>Mutations in Fas gene, Fas ligand gene, or caspase 8 or 10 genes<\/li>\n<\/ol>\n<p>Abbreviations: TCR, T-cell receptor; DNTs, double-negative T cells<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Revised diagnostic criteria for ALPS<\/strong><\/p>\n<p><strong>Required<\/strong><\/p>\n<ol>\n<li>Chronic (&gt; 6 months), nonmalignant, noninfectious lymphadenopathy or splenomegaly or both<\/li>\n<li>Elevated CD3+ TCR\u03b1\u03b2+ CD4\u2212 CD8\u2212 DNT cells (\u2265 1.5% of total lymphocytes or 2.5% of CD3+ lymphocytes) in the setting of normal or elevated lymphocyte counts<\/li>\n<\/ol>\n<p><strong>Accessory<\/strong><\/p>\n<p>Primary<\/p>\n<ol>\n<li>Defective lymphocyte apoptosis (in 2 separate assays)<\/li>\n<li>Somatic or germline pathogenic mutation in FAS, FASLG, or CASP10<\/li>\n<\/ol>\n<p>Secondary<\/p>\n<ol>\n<li>Elevated plasma sFASL levels (&gt;200 pg\/mL) OR elevated plasma interleukin-10 levels (&gt;20 pg\/mL) OR elevated serum or plasma vitamin B12 levels (&gt; 1500 ng\/L) OR elevated plasma interleukin-18 levels &gt; 500 pg\/mL<\/li>\n<li>Typical immunohistological findings as reviewed by an experienced hematopathologist<\/li>\n<li>Autoimmune cytopenias (hemolytic anemia, thrombocytopenia, or neutropenia) AND elevated immunoglobulin G levels (polyclonal hypergammaglobulinemia)<\/li>\n<li>Family history of a nonmalignant\/noninfectious lymphoproliferation with or without autoimmunity<\/li>\n<\/ol>\n<p>A definitive diagnosis is based on the presence of both required criteria plus one primary accessory criterion.
