{"id":2494,"date":"2009-03-08T17:33:30","date_gmt":"2009-03-08T17:33:30","guid":{"rendered":"https:\/\/medicalcriteria.com\/web\/hemet\/"},"modified":"2025-05-14T19:09:56","modified_gmt":"2025-05-14T19:09:56","slug":"hemet","status":"publish","type":"post","link":"https:\/\/medicalcriteria.com\/web\/hemet\/","title":{"rendered":"Diagnostic Criteria for Essential Thrombocytopaenia (ET)"},"content":{"rendered":"<div class=\"99c380e4b4a7b96c35d7ddf7dcb434e8\" data-index=\"1\" style=\"float: none; margin:0px 0 0px 0; text-align:center;\">\n<script async src=\"https:\/\/pagead2.googlesyndication.com\/pagead\/js\/adsbygoogle.js\"><\/script>\r\n<!-- MC 2019- Horizontal -->\r\n<ins class=\"adsbygoogle\"\r\n     style=\"display:block\"\r\n     data-ad-client=\"ca-pub-0127150553352455\"\r\n     data-ad-slot=\"3806776041\"\r\n     data-ad-format=\"auto\"\r\n     data-full-width-responsive=\"true\"><\/ins>\r\n<script>\r\n     (adsbygoogle = window.adsbygoogle || []).push({});\r\n<\/script>\n<\/div>\n<p><\/p>\n<div class=\"Section1\">\n<p class=\"MsoNormal\"><strong> Polycythemia Vera Study Group (PVSG) Diagnostic Criteria for Essential Thrombocytopaenia (ET)<\/strong><\/p>\n<p class=\"MsoNormal\">All of the following criteria must be fulfilled to make a diagnosis of ET<\/p>\n<ol>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">Platelet count greater than 600 x 10<sup>9<\/sup>\/L<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">Hematocrit less than 40 or normal red blood cell mass<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">Stainable iron in the marrow or normal RBC mean corpuscular volume (If these measurements suggest iron deficiency, polycythemia vera cannot be excluded unless a trial of iron therapy fails to increase the red blood cell mass into the polycythemic range.)<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">No Philadelphia chromosome or <em> bcr\/abl<\/em> gene rearrangement<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">Collagen fibrosis of the bone marrow absent or less than one third of the biopsy area without both marked splenomegaly and a leukoerythroblastic blood film<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">No cytogenetic or morphologic evidence for a myelodysplastic syndrome<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">No cause for a reactive thrombocytosis<\/p>\n<p><!--more--><\/p>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">\n<\/li>\n<\/ol>\n<p class=\"MsoNormal\"><strong>WHO Diagnostic Criteria for Essential Thrombocytopaenia (ET)<\/strong><\/p>\n<p class=\"MsoNormal\">Positive criteria<\/p>\n<ol>\n<li>Sustained platelet count &gt; 600x 10<sup>9<\/sup>\/l<\/li>\n<li>Bone marrow biopsy specimen showing proliferation mainly of the megakaryocytic lineage with increased numbers of enlarged, mature megakaryocytes<\/li>\n<\/ol>\n<p class=\"MsoNormal\">Criteria of exclusion<\/p>\n<ol>\n<li>No evidence of polycythaemia vera (PV)\n<ul>\n<li>Normal red cell mass or Hb &lt;18.5 g\/dl in men, 16.5g\/dl in women<\/li>\n<li>Stainable iron in marrow, normal serum ferritin or normal mean corpuscular volume (MCV)<\/li>\n<li>If the former is not met, failure of iron trial to increase red cell mass or hemoglobin levels to the PV range<\/li>\n<\/ul>\n<\/li>\n<li>No evidence of CML\n<ul>\n<li>No evidence of Philadelphia chromosome and no BCR\/ABL fusion gene<\/li>\n<\/ul>\n<\/li>\n<li>No evidence of idiopathic myelofibrosis\n<ul>\n<li>Collagen fibrosis absent<\/li>\n<li>Reticulin fibrosis minimal or absent<\/li>\n<\/ul>\n<\/li>\n<li>No evidence of myelodysplastic syndrome\n<ul>\n<li>No del(5q), t(3;3)(q21;26), inv(3)(q21q26)<\/li>\n<li>No significant granulocytic dysplasia; few, if any, micromegakaryocytes<\/li>\n<\/ul>\n<\/li>\n<li>No evidence that thrombocytosis is reactive due to:\n<ul>\n<li>Underlying inflammation or infection<\/li>\n<li>Underlying neoplasm<\/li>\n<li>Prior splenectomy<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p class=\"MsoNormal\">\n<\/div>\n<div class=\"Section1\"><\/div>\n<div class=\"Section1\">\n<div class=\"Section1\">\n<div class=\"Section1\">\n<div class=\"Section1\">\n<p class=\"MsoNormal\"><strong>References:<\/strong><\/p>\n<ol>\n<li class=\"MsoNormal\">Imbert M, Pierre R, Thiele J, Vardiman JW, Brunning RD, Flandrin G.
