However, &#8220;inflammatory&#8221; back pain is much less common. Back pain is considered inflammatory if four of the following five criteria are found:<\/p>\n<\/li>\n<\/ul>\n<ol>\n<li style=\"list-style-type: none;\">\n<ol style=\"list-style-type: lower-roman;\">\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Onset of back discomfort before the age of 40 years<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Insidious onset<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Persistence for at least three months<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Associated with morning stiffness<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Improvement with exercise<\/p>\n<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<ul>\n<li>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">Asymmetrical synovitis: Asymmetrical synovitis, predominantly of the lower limbs is manifested by soft tissue swelling, warmth over a joint, joint effusion, and reductions in both active and passive range of motion. As with inflammatory spinal pain, the symptoms are worse after a period of rest.<\/p>\n<p><!--more--><\/p>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">\n<\/li>\n<\/ul>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\"><span class=\"h3\">Additional criteria<\/span><span class=\"h3\">: <\/span> If a patient has one or both of the entry criteria listed above, he or she should then be evaluated for the presence of one or more of the following features:<\/p>\n<ol>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Positive family history<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Psoriasis<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Inflammatory bowel disease<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Urethritis, cervicitis, or acute diarrhea within one month before arthritis<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Buttock pain alternating between buttocks<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Enthesopathy<\/p>\n<\/li>\n<li>\n<p class=\"MsoNormal\" style=\"margin-left: 20px; margin-top: 0pt; margin-bottom: 0pt;\">Plain film radiographic evidence of sacroiliitis<\/p>\n<\/li>\n<\/ol>\n<p class=\"MsoNormal\" style=\"margin-top: 0pt; margin-bottom: 0pt;\">Importantly, blood tests, including an assessment for the presence of HLA-B27, are not part of the ESSG criteria; in addition, only the sacroiliac joints need to be evaluated radiographically.<\/p>\n<\/div>\n<p>&nbsp;<\/p>\n<p class=\"MsoNormal\"><strong>References:<\/strong><\/p>\n<ol style=\"margin-top: 0cm;\">\n<li>\n<p class=\"MsoNormal\">Gomariz EM, del M, Guijo VP, Contreras AE, Villanueva M, Est\u00e9vez EC.
