{"id":5041,"date":"2015-02-26T12:50:11","date_gmt":"2015-02-26T12:50:11","guid":{"rendered":"https:\/\/medicalcriteria.com\/web\/diagnosis-of-autoreactive-pericarditis\/"},"modified":"2026-04-06T19:26:55","modified_gmt":"2026-04-06T19:26:55","slug":"diagnosis-of-autoreactive-pericarditis","status":"publish","type":"post","link":"https:\/\/medicalcriteria.com\/web\/diagnosis-of-autoreactive-pericarditis\/","title":{"rendered":"Diagnosis of Autoreactive Pericarditis"},"content":{"rendered":"<div class=\"99c380e4b4a7b96c35d7ddf7dcb434e8\" data-index=\"1\" style=\"float: none; margin:0px 0 0px 0; text-align:center;\">\n<script async src=\"https:\/\/pagead2.googlesyndication.com\/pagead\/js\/adsbygoogle.js\"><\/script>\r\n<!-- MC 2019- Horizontal -->\r\n<ins class=\"adsbygoogle\"\r\n     style=\"display:block\"\r\n     data-ad-client=\"ca-pub-0127150553352455\"\r\n     data-ad-slot=\"3806776041\"\r\n     data-ad-format=\"auto\"\r\n     data-full-width-responsive=\"true\"><\/ins>\r\n<script>\r\n     (adsbygoogle = window.adsbygoogle || []).push({});\r\n<\/script>\n<\/div>\n<p>Recently, the term &#8220;autoreactive pericarditis&#8221; has been introduced to describe patients with identification of elements indicative of an autoimmune response who would otherwise be classed as idiopathic.<br \/>\n<!--more--><br \/>\n<strong>Diagnosis of Autoreactive Pericarditis<\/strong><br \/>\nThe diagnosis of autoreactive pericarditis is established\u00a0using the following criteria:<\/p>\n<ol>\n<li>Increased number of\u00a0lymphocytes and mononuclear cells &gt;5000\/mm<sup>3<\/sup> (autoreactive\u00a0lymphocytic), or the presence of antibodies\u00a0against heart muscle tissue (antisarcolemmal) in the\u00a0pericardial fluid (autoreactive antibody-mediated);<\/li>\n<li>Inflammation in epicardial\/endomyocardial biopsies by\u00a0P14 cells\/mm<sup>2<\/sup>;<\/li>\n<li>Exclusion of active viral infection\u00a0both in pericardial effusion and endomyocardial\/epimyocardial\u00a0biopsies (no virus isolation, no IgM-titer\u00a0against cardiotropic viruses in pericardial effusion, and\u00a0negative PCR for major cardiotropic viruses);<\/li>\n<li>Tuberculosis,\u00a0Borrelia burgdorferi, Chlamydia pneumoniae,\u00a0and other bacterial infection excluded by PCR and\/or\u00a0cultures;<\/li>\n<li>Neoplastic infiltration absent in pericardial\u00a0effusion and biopsy samples;<\/li>\n<li>Exclusion of systemic,\u00a0metabolic disorders, and uraemia. Intrapericardial\u00a0treatment with triamcinolone is highly efficient with rare\u00a0side effects.<\/li>\n<\/ol>\n<p>Pericarditis occurs in systemic autoimmune diseases: rheumatoid arthritis, systemic lupus erythematosus, progressive systemic sclerosis, polymyositis\/ dermatomyositis, mixed connective tissue disease, seronegative spondyloarthropathies, systemic and hypersensitivity vasculitides, Beh\u00e7et syndrome, Wegener granulomatosis, and sarcoidosis.
