{"id":7473,"date":"2020-03-26T20:40:05","date_gmt":"2020-03-26T20:40:05","guid":{"rendered":"https:\/\/medicalcriteria.com\/web\/?p=7473"},"modified":"2026-01-13T19:32:48","modified_gmt":"2026-01-13T19:32:48","slug":"severe-acute-respiratory-syndrome","status":"publish","type":"post","link":"https:\/\/medicalcriteria.com\/web\/severe-acute-respiratory-syndrome\/","title":{"rendered":"Case Definition for Severe Acute Respiratory Syndrome (SARS)"},"content":{"rendered":"<div class=\"99c380e4b4a7b96c35d7ddf7dcb434e8\" data-index=\"1\" style=\"float: none; margin:0px 0 0px 0; text-align:center;\">\n<script async src=\"https:\/\/pagead2.googlesyndication.com\/pagead\/js\/adsbygoogle.js\"><\/script>\r\n<!-- MC 2019- Horizontal -->\r\n<ins class=\"adsbygoogle\"\r\n     style=\"display:block\"\r\n     data-ad-client=\"ca-pub-0127150553352455\"\r\n     data-ad-slot=\"3806776041\"\r\n     data-ad-format=\"auto\"\r\n     data-full-width-responsive=\"true\"><\/ins>\r\n<script>\r\n     (adsbygoogle = window.adsbygoogle || []).push({});\r\n<\/script>\n<\/div>\n<p>Severe acute respiratory syndrome (SARS) is a viral respiratory illness caused by a coronavirus, called SARS-associated coronavirus (SARS-CoV). <!--more--><\/p>\n<p><strong>Clinical Criteria<\/strong><\/p>\n<p><strong>Early illness<\/strong><\/p>\n<ul>\n<li>Presence of two or more of the following features: fever (might be subjective), chills, rigors, myalgia, headache, diarrhea, sore throat, rhinorrhea<\/li>\n<\/ul>\n<p><strong>Mild-to-moderate respiratory illness<\/strong><\/p>\n<ul>\n<li>Temperature of &gt;100.4\u00ba F (&gt;38\u00ba C) and<\/li>\n<li>One or more clinical findings of lower respiratory illness (e.g., cough, shortness of breath, difficulty breathing)<\/li>\n<\/ul>\n<p><strong>Severe respiratory illness<\/strong><\/p>\n<ul>\n<li>Meets clinical criteria of mild-to-moderate respiratory illness, and<\/li>\n<li>One or more of the following findings:\n<ul style=\"list-style-type: circle;\">\n<li>Radiographic evidence of pneumonia, or<\/li>\n<li>Acute respiratory distress syndrome, or<\/li>\n<li>Autopsy findings consistent with pneumonia or acute respiratory distress syndrome without an identifiable cause<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Epidemiologic Criteria<\/strong><\/p>\n<p><strong>Possible exposure to SARS-associated coronavirus (SARS-CoV)<br \/>\n<\/strong>One or more of the following exposures in the 10 days before onset of symptoms:<\/p>\n<ul>\n<li>Travel to a foreign or domestic location with documented or suspected recent transmission of SARS-CoV or<\/li>\n<li>Close contact with a person with mild-to-moderate or severe respiratory illness and with history of travel in the 10 days before onset of symptoms to a foreign or domestic location with documented or suspected recent transmission of SARS-CoV<\/li>\n<\/ul>\n<p><strong>Likely exposure to SARS-CoV<\/strong><br \/>\nOne or more of the following exposures in the 10 days before onset of symptoms:<\/p>\n<ul>\n<li>Close contact with a confirmed case of SARS-CoV disease or<br \/>\nClose contact with a person with mild-moderate or severe respiratory illness for whom a chain of transmission can be linked to a confirmed case of SARS-CoV disease in the 10 days before onset of symptoms<\/li>\n<\/ul>\n<p><strong>Laboratory Criteria<\/strong><br \/>\nTests to detect SARS-CoV are being refined, and their performance characteristics assessed; therefore, criteria for laboratory diagnosis of SARS-CoV are changing. The following are the general criteria for laboratory confirmation of SARS-CoV:<\/p>\n<ul>\n<li>Detection of serum antibody to SARS-CoV by a test validated by CDC (e.g., enzyme immunoassay [EIA]), or<\/li>\n<li>Isolation in cell culture of SARS-CoV from a clinical specimen, or<\/li>\n<li>Detection of SARS-CoV RNA by a reverse-transcription-polymerase chain reaction (RT-PCR) test validated by CDC and with subsequent confirmation in a reference laboratory.<\/li>\n<\/ul>\n<p><strong>Exclusion Criteria<\/strong><br \/>\nA person may be excluded as a SARS report under investigation (SARS RUI), including as a CDC-defined probable SARS-CoV case, if any of the following applies:<\/p>\n<ul>\n<li>An alternative diagnosis can explain the illness fully<\/li>\n<li>Antibody to SARS-CoV is undetectable in a serum specimen obtained &gt; 28 days after onset of illness<\/li>\n<li>The case was reported on the basis of contact with a person who was excluded subsequently as a case of SARS-CoV disease; then the reported case also is excluded, provided other epidemiologic or laboratory criteria are not present<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>References:<\/strong><\/p>\n<ol>\n<li>Hui DSC, Zumla A.
