{"id":8107,"date":"2020-11-13T20:59:53","date_gmt":"2020-11-13T20:59:53","guid":{"rendered":"https:\/\/medicalcriteria.com\/web\/?p=8107"},"modified":"2026-01-07T18:50:07","modified_gmt":"2026-01-07T18:50:07","slug":"migraine","status":"publish","type":"post","link":"https:\/\/medicalcriteria.com\/web\/migraine\/","title":{"rendered":"Diagnostic Criteria for Migraine (ICHD-3)"},"content":{"rendered":"<div class=\"99c380e4b4a7b96c35d7ddf7dcb434e8\" data-index=\"1\" style=\"float: none; margin:0px 0 0px 0; text-align:center;\">\n<script async src=\"https:\/\/pagead2.googlesyndication.com\/pagead\/js\/adsbygoogle.js\"><\/script>\r\n<!-- MC 2019- Horizontal -->\r\n<ins class=\"adsbygoogle\"\r\n     style=\"display:block\"\r\n     data-ad-client=\"ca-pub-0127150553352455\"\r\n     data-ad-slot=\"3806776041\"\r\n     data-ad-format=\"auto\"\r\n     data-full-width-responsive=\"true\"><\/ins>\r\n<script>\r\n     (adsbygoogle = window.adsbygoogle || []).push({});\r\n<\/script>\n<\/div>\n<p>The ICHD-3 provides diagnostic criteria for the three main categories of migraine: migraine without aura, migraine with aura, and chronic migraine.<!--more--><\/p>\n<p><strong>Criteria Migraine without aura<\/strong><\/p>\n<p>At least five attacks that meet the following four criteria:<\/p>\n<ol>\n<li>Headache lasting 4-72 hours (when untreated or unsuccessfully treated)<\/li>\n<li>Headache with at least two of the following four characteristics:\n<ul>\n<li>Unilateral location<\/li>\n<li>Pulsating quality<\/li>\n<li>Moderate or severe pain intensity<\/li>\n<li>Aggravation by or causing avoidance of routine physical activity (e.g., walking or climbing stairs)<\/li>\n<\/ul>\n<\/li>\n<li>Headache accompanied by at least one of the following symptoms:\n<ul>\n<li>Nausea, vomiting, or both<\/li>\n<li>Photophobia and phonophobia<\/li>\n<\/ul>\n<\/li>\n<li>Not better accounted for by another ICHD-3 diagnosis<\/li>\n<\/ol>\n<p><strong>Migraine with aura <\/strong><\/p>\n<p>At least two attacks that meet the following three\u00a0criteria:<\/p>\n<ol>\n<li>One or more of the following fully reversible aura symptoms:\n<ul>\n<li>Visual<\/li>\n<li>Sensory<\/li>\n<li>Speech, language, or both<\/li>\n<li>Motor<\/li>\n<li>Brain stem<\/li>\n<li>Retinal<\/li>\n<\/ul>\n<\/li>\n<li>At least three of the following six characteristics:\n<ul>\n<li>At least one aura symptom spreading gradually over a period \u22655 minutes<\/li>\n<li>Two or more aura symptoms occurring in succession<\/li>\n<li>Each aura symptom lasting 5\u201360 minutes<\/li>\n<li>At least one unilateral aura symptom<\/li>\n<li>At least one positive aura symptom<\/li>\n<li>Headache accompanying the aura or following the aura within 60 minutes<\/li>\n<\/ul>\n<\/li>\n<li>Not better accounted for by another ICHD-3 diagnosis<\/li>\n<\/ol>\n<p><strong>Chronic migraine <\/strong><\/p>\n<p>Headaches (suggestive of migraine or tension headaches) on \u226515 days\/month for &gt;3 months that\u00a0fulfill the following criteria:<\/p>\n<ol>\n<li>Occurring in a patient who has had at least five attacks meeting the criteria for migraine without\u00a0aura or the criteria for migraine with aura or both<\/li>\n<li>On \u22658 days\/month for &gt;3 months, features of migraine without aura or of migraine with aura or\u00a0believed by the patient to be migraine at onset that is relieved by a triptan or ergot derivative<\/li>\n<li>Not better accounted for by another ICHD-3 diagnosis<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p><strong>References:<\/strong><\/p>\n<ol>\n<li>Headache Classification Committee of the International Headache Society (IHS) The International Classification of Headache Disorders, 3rd edition.
