Clinical suspicion of streptococcal pharyngitis (e.g., fever, tonsillar swelling, exudate, enlarged/tender anterior cervical lymph nodes, absence of cough or coryza) with

  • History of rheumatic fever or
  • Documented household exposure or
  • Positive rapid strep screen
