The new EULAR/ACR classification criteria use anti-nuclear antibodies (ANA) as an entry criterion. (Non-infectious) fever is the one new criterion. All criteria items now have individual weights (from 2 to 10) and are structured in domains, within which only the highest item is counted. There is one common attribution rule, counting criteria only if there is no more likely alternative explanation. Ten points are sufficient for classification. The new criteria have reached a sensitivity of 96.1% and a specificity of 93.4%.
Classification criteria for systemic lupus erythematosus.
Entry criterion
Anti-nuclear antibodies (ANA) at a titer of ≥1:80 on HEp-2 cells or an equivalent positive test (ever)
- if absent, do not classify as SLE
- If present, apply additive criteria
Additive criteria
- Do not count a criterion if there is a more likely explanation the SLE.
- Occurrence of a criterion on at least one occasion is sufficient.
- SLE classification requires at least one clinical criterion and ≥10 points.
- Criteria need not occur simultaneously.
- Within each domain, only the highest weighted criterion is counted toward the total score§.
